Job Summary

JOB SUMMARY:

The Social Worker/Counselor II leads the planning and coordination of patient care.  Responsible for providing social services for patients and families of inpatient behavioral health as part of a multidisciplinary team with emphasis on group and individual therapy.  Works closely with psychiatrists, as an active member of the Treatment Team by attending Care Plan meetings.  Responsible for psychotherapy group, wellness groups, psychosocial assessments and facilitating individual, marital and/or family therapy. Develops, implements, reviews and updates treatment plans as well as case management and discharge planning. 
 

Ensures assessments are completed within regulatory timeframe. Has ability to perform all requirements of the social worker/ counselor I.  Leads weekly clinical supervision meetings as assigned. Designs and implements specialized therapeutic groups for patients and families in all program areas, including education, support services and community resources.

EDUCATION / LICENSURE / CERTIFICATION / QUALIFICATIONS:

  • Graduate Degree in Social Work or Counseling, required
  • LICDC preferred, LCDC III eligible required
  • 2-5 years experience working with individuals with co-occurring mental health disorders and substance diagnosis. Experience in working in a psychiatric setting highly preferred.
  • Basic Life Support Certification required within 30 days of hire.
  • De-Escalation training within 90 days of hire; Group Psychotherapy Training

Shift and Job Schedule

Hours: Full-Time, 40 hours weekly

Shift: Days, 8:00am - 4:30pm
