Job Summary

The CHF Clinical Coordinator provides the care and service of congestive heart failure patients, promotes effective utilization and monitoring of health care resources; and assumes a leadership role with health care providers and third-party payers to achieve optimal clinical, resource, and quality outcomes.

 

The CHF Clinical Coordinator works under the direction of the Cardiac Program Director, and in collaboration with the Advanced Practice Nurse, the CHF Clinical Manager, the Heart Failure Program Medical Director, and medical staff to provide direct patient care to CHF patients enrolled in the outpatient intermittent inotropic and heart failure clinics, coordinates and teaches CHF classes, manages and tracks outcomes databases; and, interfaces with acute care, home health, and third party case managers to facilitate discharge planning and long term follow-up as needed.

 

Provides staffing to St. Joseph or St. Elizabeth CHF Clinic as needed.

 

Qualifications - Minimum

 QUALIFICATIONS:

· Experience in intermediate, cardiac or critical care required.

· Proficiency in patient education and cardiopulmonary assessment required.

· Experience assessing and caring for Class I to IV Heart Failure patients preferred.

· Experience with IV access devices preferred.

· Computer literacy; proficient in use of Microsoft Office, HBOStar required.  Midas preferred.

 

EDUCATION

· Graduate of an accredited School of Nursing, BSN preferred.

· Current R.N. license in Ohio.

· CPR certified, ACLS required.

 

ESSENTIAL FUNCTIONS:

Professional Practice:

Maintains expertise in clinical field. Seeks learning opportunities to enhance knowledge base.

Maintains confidentiality including, but not limited to, patients and families.

Represents the CHF Program and Mercy Health with a positive and professional attitude toward physicians, patients, insurers, and outside reviewers.

Maintains a positive attitude in motivating the delivery of quality patient care within Mercy Health.

Makes recommendations for changes and/or improvements in a tactful, constructive manner to the appropriate persons.

Participates in program planning and quality improvement activities. Develops and presents appropriate educational programs for identified departments.

 

Clinical Practice:

Acts as advocate in identifying patient/family needs, system barriers, and resolution.

Collaborates with advanced practice nurse, medical director, and primary care physician to provide patients to appropriate level of care.

Provides direct patient care to inotropic and CHF clinic patient safely, efficiently, and within scope of practice.

Responsible for initiating, implementing and educating CHF patients in oral medication titration according to the practice guideline under the direction of the referring physicians.

Performs telephone surveillance and monitors long-term outcomes; demonstrates appropriate triage and critical thinking skills.

Develops and presents CHF classes.

Monitors and evaluates patient’s health care needs and recommends options in the continuum of care to optimize long-term outcomes.

Coordinates health risk and functional status appraisals to assess wellness needs.

Provides condition specific and general health information to patients and families using all currently available media.

Reviews and assesses health needs at each interaction, updating and clarifying information as appropriate.

Works with multidisciplinary team to organize, coordinate, and provide patient and family education related to illness and expected outcomes in order to promote a sense of security and satisfaction.

Provides patient and family education related to discharge options, community resources and financial assistance.

Works within the constraints of the payer and contractual agreements. If not possible, serves as an advocate and utilizes the appeals process.

 

ORGANIZATIONAL AND STATISTICAL ACTIVITIES:

Effectively organizes daily work to meet priorities and to accommodate fluctuations in workload.

Inputs all data into computer as required.

Documents pertinent information.

Maintains statistics and generates reports as requested.

Is accurate, complete, and timely with data collection.

Collects and assesses quality data.

Assume responsibility for collection and reporting of pathway outcome data as assigned.

 

COLLABORATION:

Develops and promotes effective communications with staff, physicians, patients/families, and with other members of the healthcare team.

Communicates effectively in oral and written format.

Initiates and maintains effective working relationships with physicians and other health disciplines.

Demonstrates the ability to function as a team member.

Collaborates with finance to identify and resolve billing issues.

Participates in multidisciplinary team meetings to address specific patient needs/complex planning.

Actively participates on committees, task forces, and work groups as appropriate.

Participates in CQI Teams, analyzes data, and presents outcomes to the appropriate departments.

Collaborates with Mercy Health and health care team-members and organizational education to develop and present educational programs.

Hours

Hours per pay period: 80

Hours: 7:00am to 3:30pm

Hours may need to be flexed based on patient care need.
