As a member of the Interdisciplinary Group, the Home Health Aide provides personal care services to hospice patients. The Patient Care Assistant performs related tasks primarily in the Hospice Center setting, and as assigned in the community and nursing facility settings, which are in accordance with the Plan of Care, and are assigned by the RN/LPN.
Qualifications - Minimum
Successfully completed a nurse assistant/Home Health Aide certification course or is in progress to obtain HHA certification. (STNA)
Ability to make meaningful observations and write concise, accurate reports.
Ability to recognize the needs of people and to maintain good relationships in dealing with them.
Maintains annual CPR certification.
Previous experience as a home health aide/ STNA.
High school graduate.
Effective oral and written communication skills.
Normal patient care environment with little exposure to noise, dust, temperature, and the like.
Very frequent exposure to moderately adverse working conditions due to performance of certain distasteful patient care activities.
Ability to concentrate and pay close attention to detail fifty percent or more of work time.
Ability to stand, bend and walk more than eight percent of work time.
Directly accountable to the Director or designee.
Hours per pay period: 72
Hours: 11:00am to 11:30pm
