Position is for either an RN Case Manager or a Licensed Social Worker
Job Summary: Collaborates with physicians and clinical staff in initiation, development, coordination and integration of ongoing clinical assessment, patient care planning, patient teaching, assessment of patient progress through the continuum of care and evaluation of discharge requirements necessary for the achievement of determined outcomes. This responsibility is assumed within the context of appropriate allocation and utilization of resources and quality of care, collaborating with professional hospital and community disciplines to support the patient along the continuum of care.
Hours: Full-Time, 40 hours a week, 8:30a-5p, Every 4th Saturday, Every seventh Saturday and Sunday, one holiday per year
Experience in Case Management and Utilization Review in the acute care setting. Knowledge of Milliman and Interqual criteria. Knowledge of Epic and basic computer skills.
RN with a BSN or Licensed Social Worker with Social Work experience
