The Licensed Practical Nurse (LPN) functions as a member of the patient care team under the supervision of the Registered Nurse (RN). The LPN assists in the delivery of patient care by data collection, observation, implementation, and evaluation. As a member of the healthcare team, the LPN will employ interpersonal and teamwork skills.
Essential Duties and Responsibilities:
Performs patient data collection and observation considering patient's age group, severity of condition/disease to include physical, psychosocial, cultural, environmental, self-care, educational, and discharge planning needs.
Implements patient care in a safe, knowledgeable, skillful, consistent, and continuous manner. Contributes to the plan of care with input from the patient/family and other members of the healthcare team.
Evaluates effectiveness of patient care and the processes for achieving positive patient outcomes.
Recognizes and reports significant changes in the patient's condition, and contributes to plan of care revision bases on ongoing evaluation.
Adheres to the safety policies and procedures of the organization. Recognizes and reports potential safety issues.
Demonstrates the ability to use bio-medical equipment
Participates in supporting the requirements of regulatory bodies i.e. O.D.H./The Joint Commission.
Contributes to the achievement of the resident's goals.
Adheres to therapies plan of care.
Contributes to the educational needs of the resident for independence at home.
Performs all duties, with or without accommodations, listed in the Physical Demands section of this job description.
Supports the healthcare team by participating patient care and unit activities.
Participates in performance improvement activities and peer review.
This job has no supervisory responsibilities.

Certificates, Licenses, Registrations:
Must have licensure.
Education and/or Experience:
High School Diploma or GED and Certificate of accredited allied health program
Years of Experience: 1-3 preferred
Graduate of accredited allied health program.
One to two years of Medical/Surgical Nursing preferred, or experience in specialty.
