Under the supervision, works collaboratively with the Emergency Department Physician, Psychiatrists and local mental health centers staff to facilitate proper patient referral and disposition in an efficient and timely manner. The least restrictive environment within which the patient can achieve resolution of his/her problems is recommended, whether that is inpatient admission or referral to an appropriate outpatient service.
Essential Duties and Responsibilities:
Completes the patient's social history and biopsychosocial assessment, including assessment of the patient's aftercare needs. Assessment includes specific treatment goals of the patient.
Investigates case situations presenting to the Emergency Department and presents information to the Emergency Department Physician, Psychiatrist and/or other members of the health team on patient's biopsychosocial information.
Provides basic education to the patient/family to help reduce fear and other attitudes obstructing acceptance of psychiatric care and continuation of treatment.
Serves as a link between patient/family and Emergency Department and Physicians.
Develops and negotiates with the client treatment plans which address the fundamental needs of the client and facilitate improvement in their mental status.
Attends workshops/seminars to improve social work skills.
Participates in Performance Improvement activities as requested by the Manager or Coordinator.
Assists with biopsychosocial assessments as needed on inpatient units.
Performs Utilization Review pre-certifications from the Emergency Department as needed and places "notes" in computer.
Abides by Professional Code of Ethics of Ohio Counselors and Social Work Board.

Qualifications - Minimum
Professional preparation in Social Work, minimum requirement Bachelor’s Degree. At least two years of clinical experience working with client systems, preferable in mental health.
Licensed as a Social Worker by the State of Ohio.
Hours per pay period: 32
Hours: 4:00pm to 12:00am
Rotational weekends and holidays.
Weekend work expected.
