Main Accountabilities Statement:
Acts as a liaison to promote a coordinated interdisciplinary working relationship between physicians, nursing service, other health team members and the patient and to provide quality patient care in accordance with hospital mission and values. Makes professional judgments, counsels and teaches patients, patient’s families and co-workers. Compiles with appropriate section of the Medical Staff By-Laws and nursing policies established. Demonstrates clinical competency in relation to the components of the nursing process. Serves as a clinical expert, role model and resource to the team. Renders direct and indirect nursing care using the nursing process. Works collaboratively with the Nurse Managers of their unit for change, evaluates practical standards and problem resolution.
I. Performs a health status assessment (H&P) and initiates and follows through with appropriate nursing and medical action to meet patient care goals.
A. Interviews patient to obtain a health history.
B. Performs physical examination including all body systems and records on patient record.
C. After assessing patient and analyzing/interpreting information, notifies physician of any pertinent findings and obtains orders.
D. Performs vaginal examination to obtain pap smears and cultures when trained and proficient.
E. Performs discharge assessment/evaluations of Ambulatory Care Center patients from childhood through adulthood, if requested.
II. Acts as liaison to promote coordinated interdisciplinary working relationship between physicians, nursing and other health care team members and the patient.
A. Utilizes rounds with physicians to ensure optimum patient-centered care.
B. Evaluates patients, identifies problems and assists in coordination of patient care plans.
C. Writes and signs appropriate progress notes on patient record.
III. Institutes appropriate measures and orders (according to standing protocol) to meet patient needs in accordance with individual physician preference (dealing with both acute and chronic illness).
A. Evaluates patient and identifies problem.
B. Writes orders (using approved protocols) as indicated in collaboration with the attending physician.
C. Performs other procedures duly authorized by the Medical Staff and hospital administration.
IV. Provides health counseling.
A. Provides health care counseling and guidance in such areas as family planning, medication use, self-care when ill, abnormal and normal function of the human body, weight reduction and smoking cessation and stress management.
B. Utilizes additional resources as indicated, both hospital and community, i.e., Mental Health consult, EAP, WIC, etc.
IV. Assumes responsibility for job performance and for ensuring continued professional growth.
A. Delivers patient-centered care based on standards of nursing practice and in accordance with hospital policies, procedures and nursing process.
B. Identifies goals for self-improvement and develops action plan to achieve same.
C. Demonstrates support of quality improvement efforts and monitors and utilizes results of internal and external indicators.
D. Utilizes results of NCG Survey, Maryland Quality Indicators and employee surveys to improve patient care.
E. Actively seeks out and participates in learning activities and professional organizations to promote own professional/personal growth.
F. Acts as a mentor in the professional development of his/her peers/nursing students through patient care conferences, sharing knowledge obtained through experience, seminars and conferences, formal education presentations, etc. Assists in the orientation of new employees.
V. Demonstrates sense of responsibility for patient teaching by awareness and identification of patient’s learning needs for patient and significant others.
A. Assists with teaching patient and/or significant others how to care for self after hospital stay and documents same.
B. Recognizes and uses opportunities for teaching of patients and significant others.
VII. Actively pursues and initiates other duties as need arises.
A. Participates in unit activities, shared governance councils and other hospital committees.
B. Actively seeks out and participates in community outreach.
C. Collaborates with Nurse Manager, Organizational Development and other resources in the provision of education programs.
D. Demonstrates competency in unit specific activities, skill performance and equipment use.
I. Assumes additional responsibilities and performs other related duties as indicated.
Qualifications - Minimum
Graduate from an accredited school of nursing and current licensure by the Ohio State Board of Nurse Examiners is required.
Certification is required to practice as a nurse practitioner.
Must maintain certification every five years through continuing education and community service.
If hired prior to 1997, BSN and Certificate by the State is required.
If hired after 1997, Master’s degree and Certificate by the State is required. BLS and ACLS certification is required.
EXPERIENCE: A minimum of three years experience as a registered nurse is preferred.
KNOWLEDGE AND SKILL:
