Mercy Health’s Behavioral Health Institute seeks to enhance population health through the institution of standardized, evidence-based, and integrated behavioral health treatment in the primary care setting. We endeavor to provide quality, affordable care and to improve the emotional health and well-being of those whom we serve. This position will provide behavioral health consultation utilizing an integrated, team-based primary care model to children, adolescents, adults and families in order to improve general health status and achieve various health-related and psychosocial goals.
Provide comprehensive assessment and diagnosis of behavioral health clients.
Provide effective treatment planning and assisting clients in successfully achieving goals.
Evaluate crisis situations and apply appropriate interventions.
Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration.
Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management and patient education to improve self-management of chronic disease.
Provide assessment, consultation and brief (20 minute) intervention for psychological/psychiatric problems and/or disorders.
Teach patients, families and staff care, prevention and treatment enhancement techniques.
Monitor the site's behavioral health program, identifying problems related to patient services and making recommendations for improvement.
Participates in Quality Improvement (QI), Quality Assurance (QA) AND Continuous Quality Improvement Activities (CQUI) as appropriate.
Qualifications - Minimum
Specialty/Major: Social Work
Required: Licensed in the state of practice. Licensed Independent Social Worker (LISW) in the state of Ohio. Licensed Clinical Social Worker (LCSW) in state of Kentucky.
Must be licensed as an independent social worker, two years relevant experience.
Preferred experience working in a primary care setting.
Hours per pay period: 80
Hours: 8:00am to 4:30pm
Grant Funded Social Work position working in the Mother Child Dependency Program.
Provides Home Visitation program to Opiate Positive Pregnant Women.
