SUMMARY OF JOB RESPONSIBILITIES:
The Core Nurse is responsible for overseeing daily operations and providing support to multidisciplinary clinical teams to ensure quality patient care is delivered to a specific geographical region.
Good interpersonal skills in relationship to patients, caregivers, other health care professionals outside the organization, and other employees within the agency.
Demonstrates ability to prioritize and multitask.
Demonstrates leadership and team building skills necessary to case manage the patient while coordinating the care with other health care professionals.
Knowledge of community resources necessary for discharge planning and to utilize appropriate resources to reach the patient’s goals.
Clinical thinking necessary to evaluate, process and reach conclusions related to the observations made during the assessment process.
Qualifications - Minimum
Registered Nurse licensed in Ohio.
Current State drivers license in good standing.
Reliable transportation and current liability insurance on vehicle to be used for home visits
Previous experience in home health care is strongly preferred.
Minimal clinical experience 2 years (acute care or home care)
Excellent assessment skills required.
Knowledge of Home Health regulatory Guidelines (Conditions of Participation, HM 11, JCAHO/CHAP) plus practice act guidelines by discipline.

Safety Enhancement (Patient & Employee)
PRINCIPAL JOB ACCOUNTABILITIES:
Overseeing daily scheduling of patients.
Reporting lab results.
Triaging of patients, caregivers, physician concerns and issues.
Scheduling of staff to ensure adequate staffing levels.
Assisting duties of Clinical Nurse Manager as deemed necessary.
As assigned, able to fulfill the duties of the Clinical Nurse Manager in he/her absence.
Hours per pay period: 0
Hours: 8:00am to 4:30pm
Available for 5 days per month inclusive of 1 weekend 1 holiday.
