Job Summary

Hospice nursing is a rewarding path in the field of nursing. As an RN Case Manager, you will play a pivotal role in the care of terminally ill patients in their home while being part of a highly qualified and committed inter-disciplinary healthcare team.  As the RN Case Manager, you will provide in-home nursing assessments, planning and coordination of the patient’s plan of care with other team members to ensure that symptoms are managed and that the patient/family needs are met.  The honor of ensuring quality care and sharing in this final life journey for others’ loved ones is the experience that awaits a successful compassionate nursing professional.


Qualifications - Minimum

Associate Degree in Nursing (ADN) from an accredited school of nursing

Kentucky Registered Nurse License; or active compact state-issued license

Basic Life Support (BLS) through AHA 

Two years of experience in acute care

Qualifications - Preferred

Bachelor of Science in Nursing (BSN) from an accredited school of nursing

Certified Case Manager (CCM)

One year of hospice care experience

Physical Demands

Works within patient care units and other areas as assigned. Working beyond  normal working hours, on weekends, and holidays, and in other positions temporarily, when necessary.  Working throughout the facility moving around intermittently thoughout the shift.  Frequent interruptions.


8am - 4:30pm

Shift and Job Schedule

Day shifts with rotating schedules
