Job Summary

The Licensed Practical Nurse provides care in an atmosphere sensitive to each person’s physical, emotional, social, and spiritual needs in accordance with the mission of St. Rita’s Medical Center and within the scope of the Ohio Nurse Practice Act. The Licensed Practical Nurse functioning in the spirit of teamwork is accountable for the delivery of nursing care utilizing the nursing process using evidence based standards, policies, procedures, and guidelines of the organization. The Licensed Practical Nurse contributes to the interdisciplinary patient care. The Licensed Practical Nurse will incorporate the core values of St. Rita’s Medical Center (Excellence, Human Dignity, Justice, Mercy, Sacredness of Life and Service) and the American Nursing Association Code of Ethics for nurses into their daily practices.

 

Essential Duties and Responsibilities:

Under the direction of the Registered Nurse, the Licensed Practical Nurse will:

Assessment:  The Licensed Practical Nurse shall contribute observations to the nursing assessment of the client.  The Licensed Practical Nurse shall accurately and timely:

A)  Collect and record objective and subjective data and observations about the clients condition; and

B)  Report and record objective and subjective data as appropriate to other members of the health care team

Planning:  The Licensed Practical Nurse shall accurately and timely contribute to the development of the nursing component of the clients plan of care.

Implementation:  The Licensed Practical Nurse shall accurately and timely implement the nursing plan of care, which may include:

A)  Administering medications and treatments prescribed by:

     1)  1) A licensed physician, dentist, optometrist, or podiatrist

     2)  An advanced practice nurse approved under section 4723.55 of the revised code.

     3)  A certified nurse-midwife, certified nurse practitioner, certified registered nurse anesthetist, or clinical nurse specialist approved under section 4723.41 of the revised code

     4)  A physician assistant in accordance with chapter 4730 of the revised code and the rules of the ohio state medical board.

B)  Giving direct nursing care at the direction of:

     1)  A registered nurse

     2)  A licensed physician, dentist, optometrist, chiropractor, or podiatrist; or

     3)  An advanced practice nurse, certified nurse-midwife, certified nurse practitioner, certified registered nurse anesthetist, or clinical nurse specialist;

C)  Assisting with care of the client at the direction of:

     1)  A registered nurse;

     2)  A licensed physician, dentist, optometrist, chiropractor, or podiatrist; or

     3)  An advanced practice nurse, certified nurse-midwife, certified nurse practitioner, certified registered nurse anesthetist, or clinical nurse specialist;

D)  Collaborating with other nurses and other members of the healthcare team; and

E)  E) Delegating delegable nursing tasks in accordance with chapter 4723-13 of the administrative code, or delegating giving oral or applying topical medications in accordance with chapter 4723-21 of the administrative code, or chapter 4723-22 of the administrative code.

 

F) Under the direction of the Registered Nurse, the Licensed Practical Nurse will:

 

Call or fax prescriptions to pharmacies as indicated by provider/practitioner

Pre-authorize prescriptions, outpatient testing communicates information to patient and document in the patients record

Perform accurate, legal, and ethical documentation at all times

Able to pull up patient health information history through CarePATH/power chart for physician/practitioner as needed

Makes outbound calls as needed to ancillary departments, physicians and other agencies

Travel to outreach offices for coverage as needed and when applicable

Scheduling appointments, assisting with form completion, insurance verification entering patient data and demographics into computer database

Teach patient/significant other appropriate health information in a timely manner and share written informational material

Perform direct patient care, utilizing established procedures, policies, and standards

Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals

Demonstrate ability to handle emergency situations in a prompt, precise, and professional manner

Schedules tests as directed by provider

Implements care in a safe, knowledgeable, skillful, consistent and continuous manner

Evaluates the effectiveness of equipment and processes for achieving positive patient outcomes

Adheres to the safety policies and procedures of SRPS

Recognizes and reports potential safety issues

Demonstrates ability to utilize biomedical equipment

Must be trained and competent in the Scheduling, Registration, MA/Nurse 100 & 200 functionality of CarePATH

Maintains confidentiality and actively supports patient/family rights

Supports and contributes to organizational cost-saving initiatives

Serves as a patient advocate

Administer medications according to practice policies and procedures

Maintain a clean, neat, and safe environment for patient and staff

Attends staff meetings as directed by the practice manager/supervisor

Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner

Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated, triages phone calls as necessary

Demonstrate sound judgment by taking appropriate actions regarding questionable findings or concerns

Ensure compliance with regulatory standards

 

Evaluation:  The Licensed Practical Nurse shall accurately and timely:

A)  Contribute to the evaluation of the client's response to nursing interventions.  As appropriate, the evaluation shall involve the client, family, significant others, or other members of the health care team:

B) Document and communicate the client's responses to nursing interventions to appropriate members of the health care team; and

C)  Contribute to the revision of the nursing component of the client's plan of care on the basis of the evaluation.

Education and/or Experience:

Education:

Associates Degree

 

Years of Experience: 1-3

 

Graduate of accredited allied health program.

One to two years of Medical/Surgical Nursing preferred, or experience in specialty.

Demonstrates assertive communication skills. Develops and maintains effective communication with internal and external customers. Ability to read, analyze and interpret professional journals, technical procedures, or governmental regulations. Ability to write reports and document observations.

Microsoft Windows

 

Works within a windows environment and is familiar with the functionality of the windows tools.

 

Has proficiency working with the applications found within Microsoft Office Suite, including Microsoft Word, Excel, PowerPoint and Access.

Internet/Intranet: Has the ability to access the organization’s intranet and is familiar with the different content and page layouts.  Can access information from the different sites within the web page.  Can access the internet and search for relevant material as it relates to the current job.

Must be able to access the appropriate applications.  Must be able to use the functionality of the applications for the successful completion of the employee’s essential job requirements.  Maintains the security of the system and access codes per hospital policy.

Weekend, 12 hour shifts, rotating shifts at Delphos Urgent Care, East Side Urgent Care and Luke West Side Urgent Care.
