Job Summary

Functioning within the guidelines set forth in the philosophy and mission of Mercy Health Home Care Services, the Licensed Social Worker facilitates continuity of patient care by providing professional social work services to patients, family members and groups, including psycho-social assessments, crisis intervention and referral service to community agencies.  Delivering these services to home care patients require collaboration with physicians, nurses and other members of the health care team to ensure that the needs of patients are met.  The Licensed Social Worker functions within the framework of social work practice in consultation with referral services and as liaison to community agencies.

 

The Licensed Social Worker works under the supervision of the Clinical Manager assigned and the Assistant Clinical Manager adheres to current standards, guidelines and Code of Ethics of the National Association of Social Workers and must meet requirements for State Licensure for social workers in Ohio. Consults with Mercy Health LISW for matters related to practice/complex patient problems.

 

Qualifications - Minimum

Education:

Graduate of an accredited college with a Bachelor of Social Work degree required.

 

EXPERIENCE:

Minimum of one (1) year experience in acute or home health care setting.

 

Hours

Hours per pay period: 80

Hours: 7:30am to 4:00pm

Weekends and holiday rotations.
