Job Summary

The Licensed Practical Nurse provides care within the scope of the Ohio/Kentucky Nurse Practice Act. The Licensed Practical Nurse is accountable for the delivery of nursing care utilizing the nursing process, evidence-based standards, policies, procedures and guidelines of the organization. The Licensed Practical Nurse will incorporate the American Nursing Association Code of ethics for nurses in daily practice.



Able to read, analyze and interpret professional journals, technical procedures and understand governmental regulations. Ability to write reports and use proper grammar when documenting.

Requires full range of body motion including handling and lifting patient, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extended periods of time. Frequently lifts and carries items weighing up to 100 pounds (with assistance). Requires corrected vision and hearing to normal range. Requires working under stressful conditions and irregular hours. Exposure to communicable diseases and/or body fluids.


Qualifications - Minimum


Certificates License/ Registrations:  Current unencumbered applicable state license required.   Basic Life Support Certification required. De-Escalation Training required within 90 days of hire.


Education and /or Experience: Graduate of an approved practical nurse program. AAPT level II within one year of hire. Experience in working in a psychiatric setting highly preferred.


Special physical requirements: Sits and stands throughout the day. Physical management of patients in crisis situation may be necessary.

Under the direction of the Registered Nurse, the Licensed Practical Nurse shall:


Assess: The Licensed Practical Nurse shall contribute observations to the nursing assessment of the behavioral health patient.  The License Practical Nurse will accurately and timely:


Collect and record objective data and subjective about the patient’s condition; and


Report and record objective and subjective data as appropriate to the other members of the healthcare team.


Demonstrate knowledge of the principles of growth and development over the lifespan.


Plan: The Licensed Practical Nurse will accurately and timely contribute to the development of the nursing component of the patient’s care plan.


Implement: The Licensed Practical Nurse will accurately and timely implement the nursing plan of care, which may include:

Administrating medications and treatments accurately as prescribed by the Physician, Physician Assistant or Certified Nurse Practitioner.


Administering nursing care under the direction of a Registered Nurse.


Collaborating with other members of the treatment team.


Complete 15 minutes safety checks.


Evaluate: The Licensed Practical Nurse shall accurately and timely contribute to the evaluation of the patient’s response to the nursing interventions.


Document and communicate the patient response to nursing interventions to the appropriate members of the healthcare team.

Other duties as assigned by Director or Manager.



Hours per pay period: 64

Hours: 6:30am to 3:00pm

Weekends and holidays are rotated.
