Job Summary

Employee/Pt Discharge Pharmacy

Reports to Manager, Pharmacy Services; under the general supervision and work direction of the staff pharmacists. Fills patient orders, floor stock, ancillary department supplies and unit dose medication carts for terminal check by registered pharmacist. Enters charges and credits onto patient profile via CRT. Delivers patient medications from pharmacy, returns orders and other pharmacy-bound materials. Maintains drug stock within pharmacy.  Prepackages drugs using established procedures.

Qualifications - Minimum

EDUCATION:

High school diploma or equivalent (e.g. GED) required.

Currently enrolled in a degreed pharmacy program

Must have valid Ohio Intern License

 

EXPERIENCE:

Previous experience in hospital or community pharmacy highly desirable.

Familiarity with basic equipment such as computer, electronic calculator, etc.

Previous computer experience desirable.

 

KNOWLEDGE AND SKILLS:

Knowledge and skill required to perform the job responsibilities for both technical and dimensional content.

 

Follows general instructions. Understands several defined work routines. Reads various references, extracts information and applies basic arithmetic functions. May operate basic equipment. Knowledge of basic medical and pharmaceutical terminology and basic pharmacy calculations/equivalents required.

The degree to which judgment and analysis must be exercised in planning, investigating issues, and evaluating alternative solutions to the challenges posed by the job.  Deals with the nature of the decisions, the variety of problems, and how decisions are made in context of the typical requirements of the job.

SCOPE OF IMPACT/ACCOUNTABILITY:

The potential impact of actions on results of HM Health Services and the job’s accountability for achieving results.

May provide information to decision-making processes affecting work area. While all technical functions are checked by a registered pharmacist, employee is accountable for maintaining considerable concentration and accuracy while performing job functions.

The nature and frequency of interpersonal human relations skills required by the job.  The extent to which the job requires cooperation and tact in meeting or influencing others by phone, correspondence, or personal contact.

PRINCIPAL JOB ACCOUNTABILITIES:

Delivers medications from pharmacy to nursing units; returns items from nursing units as needed.

Leaves pharmacy within 10 minutes of scheduled rounds

Delivers medication to proper unit, picks-up orders, requisitions and other materials

Rounds completed within 15-20 minutes

Deliver unit dose cassettes each morning; properly exchange supplies; handle “transfer” items properly

 

Proper placement of refrigerated items:

Assures ADT notification handling within pharmacy.

Properly completes ADT notices in timely manner

Properly labels patient bins affected by ADT

 

 

Floor stock processed for nursing units/ancillary/satellite areas:

Accurately fills requisitions; initials and dates sheets

 

Maintains proper stock levels in picking areas; removes any outdates discovered has pharmacist check work when completed checked items delivered by end of shift enters low stock onto Want Book for ordering

 

Unit Dose carts filled for patient medication administration:

All cassettes identified with patient name label cassettes accurately filled cassettes filled in a timely manner

 

Fill lists properly indicated with charges/credits fill lists initialed and dated when completed; left for RPh check

 

Replenishes stock in picking areas; removes outdates

 

Maintains picking areas in neat, organized manner

 

Enters low stock onto Want Book for ordering

 

Places checked refrigerator boxes into refrigerator until cart exchange

 

Patient charges/credits accurately processed:

Accurately posts charges and credits onto CRT

 

Assures patients are charged for proper drugs/quantities

 

Processes in timely manner after cart fill

 

Controlled Substances handled appropriately:

Performs sorting and filing functions as needed

 

Delivers controlled substances to nursing units after pharmacist fills orders; obtains nursing signatures prior to leaving nursing unit                                      

 

Picks-up any expired/unusable narcotics from nursing units and returns to pharmacist for disposal

 

Provides general assistance to pharmacist staff:

Assist pharmacist in new order area as needed

 

Accurately set-up orders with correct medication

 

Type labels in proper format for bulk dispensing’s

 

Performs basic nursing unit inspection procedure functions

 

Restocks EMS drug and IV boxes:

Correct items replaced into box per EMS flowsheet

 

New flowsheet initiated for restocked box

 

New lock placed with box; serial # marked on flowsheet

 

EMS drug supply area kept stocked

 

Restocked boxes left for pharmacist check

 

Wholesaler order processed and received correctly:

Places wholesaler order via computer or Telxon unit in timely manner

 

Receives wholesaler order - confirms receipts vs. invoices

 

Price-stickers all receipts

 

Receipts are shelved, rotating older stock

 

Invoices, packing slips, etc. placed in manager’s invoice box for payment approval

 

Notify manager of shorts, breakage, etc.

 

Prepackaging of bulk drugs performed:

Drugs prepackaged properly; labeled correctly

 

Prepackaging control logs maintained as required

 

All prepackaged items submitted for pharmacist check prior to putting into stock

 

Performs basic I.V. room functions when assigned to area:

Uses appropriate aseptic technique

 

Able to generate labels and charges for IVPB’s and LVP’s

 

Able to admix IVPB’s, LVP’s, TPN solutions, basic miscellaneous sterile products

 

Able to function in Employee Prescription area as assigned:

Receive prescription; screen for legible name; verify prescription benefit coverage

 

Retrieve profile; post prescription properly

 

Retrieve and count medications; put in proper container; leave for RPh check

 

Provide proper prescription to patient upon pickup

 

Refers questions re: drug therapy to RPh counseling as appropriate.

 

Performs miscellaneous duties and follows guidelines:

Never releases any drug prior to pharmacist check

 

Uses appropriate judgment; refers problems to pharmacist if needed

 

Organizes time effectively to meet changing workload demands

 

Demonstrates consistent level of performance

 

JOB ACCOUNTABILITIES – Other:

Able to perform basic tasks of other non-professional staff during low-staff periods (PTO, etc.) w/boundaries of education and training.

 

Answers telephone using proper protocol; refers calls to pharmacist when appropriate.

 

Able to handle work with frequent interruptions.

 

Reports statistics for workload monitoring as required.

 

Assists in training of new employees and pharmacy students assigned to department.

 

Wears prescribed uniform per Departmental policy.

 

Performs assigned stock section checks as assigned.

 

Knows and follows hospital and departmental procedures; knows location of hospital and departmental Policy/Procedure manuals.

 

Projects a professional image for department
